“"Whether the tipping and righting of the car is to be regarded as the `accident' or whether the heart strain in righting the car is taken to be the `accident' we do not regard as a matter of decisive importance. Certainly the resulting heart strain under the circumstances would be an unexpected, undesigned, and unlooked for mishap. It is the causal connection between the accident and the injury, in this case the death, that is controlling.
