A person who holds a permit under this chapter shall submit an annual report not later than December 15 to the department. The report must be made on a form prescribed by the department and contain information pertaining to (1) species;
(2) the brood stock source;
(3) the number, age, gender, and size of spawners;
(4) the number of eggs collected and juveniles produced; and
(5) the number, age, gender, and size of harvested shellfish attributable to releases by the permit holder.
