The State Board of Health may establish the fees to be charged by the Department of Health to defray the cost of administering and enforcing this subchapter, as follows:
(1) The testing fee not to exceed the cost of administering the National Registry of Emergency Medical Technicians examination;
(2) The licensure fee for emergency medical services personnel, which shall not exceed twenty dollars ($20.00).
(3) Ten dollars ($10.00) of the licensure fee shall be credited to the Emergency Medical Services Revolving Fund.
(4) The licensure shall be valid for two (2) years;
(5) The biennial renewal of the emergency medical services personnel licensure, which shall not exceed twenty dollars ($20.00). Ten dollars ($10.00) of the biennial renewal shall be credited to the Emergency Medical Services Revolving Fund;
(6) The issuance and annual renewal of an operational permit for each ambulance service, which shall not exceed fifty dollars ($50.00);
(7) The annual inspection and permitting of emergency vehicles, which shall not exceed five dollars ($5.00) per vehicle; and
(8) The issuance and renewal of an operational license for each air ambulance service, which shall not exceed one hundred dollars ($100).
