Expiration of certificate — Renewal

(1) A certificate expires on the second anniversary of its effective date unless the certificate is renewed for a two-year term as provided in this section.

(2) Before the certificate expires, a certificate may be renewed for an additional two-year term if the applicant: Otherwise is entitled to the certificate;

(3) Pays the State Board of Health the renewal fee set by the board by rule; and

(4) Submits to the board: A renewal application on the form that the board requires; and

(5) Satisfactory evidence of compliance with any requirement of this subchapter for certificate renewal.
