(1) The return envelope for an absentee voter’s ballot must have printed on its face a self-affirmation substantially in the form provided in section 1-13.5-605 (1). (2) If applicable, the self-affirmation provided in section 1-13.5-605 (2) may be substituted for the self-affirmation in section 1-13.5-605 (1).
