C.R.S. § 11-49-112

(1) Every provider shall maintain a register setting forth the following facts concerning each person residing in the life care institution: (a) Name; (b) Last previous address; (c) Age; (d) Nearest of kin, if any; (e) Mother’s maiden name; (f) The person responsible for each resident’s care and maintenance; (g) Such other data as the commissioner may reasonably require.
