(1) An adult who has decisional capacity may execute a medical orders for scope of treatment form. (2) Except as provided in section 15-18.7-110 (3), the authorized surrogate decision-maker for an adult who lacks decisional capacity may execute a medical orders for scope of treatment form for said adult.
