Known as the Prepaid Limited Health Service Organization Act
The act spans §§ 636.002 to 636.067 (55 sections).

Every prepaid limited health service organization subject to this act must pay to the office the following fees:
(1) For filing an application for a certificate of authority or amendment thereto: $500.
(2) For filing each annual report: $200.
(3) For each renewal of certificate of authority: $500.
