Failure to give notice of change of address - penalty

Known as the Illinois Dental Practice Act

The act spans §§ 225-25-1 to 225-25-9 (85 sections).

Any failure, neglect or refusal on the part of any person obtaining a license to practice dentistry from the Department to notify the Department of any change of address within 90 days thereof shall effect a forfeiture of such license, and no license, when once forfeited, shall be restored, except upon payment to the Department of the fee for such neglect, failure or refusal.
