county, dated (insert YES date), to be known as (insert here the name of the proposed district as -------------------- entered in the order of the judge of the circuit court) and described as NO follows: (insert description of the proposed district as entered in the order of the judge of the circuit court)?" -------------------------------------------------------------- The clerk of the circuit court of the county shall cause a statement of the result of the referendum in the proposed district to be filed in the records of the circuit court of the county, and if a majority of the votes cast in the proposed district upon the question is found to be in favor of the organization of a forest preserve district, the proposed forest preserve district shall thenceforth be deemed an organized forest preserve district under this Act.
