Mass. Gen. Laws ch. 112, § 151

A person who desires to be licensed and registered as an acupuncturist shall apply to the committee in writing on an application form prescribed and furnished by the committee. He shall include in his application statements under oath satisfactory to the committee, showing that he possess the qualifications preliminary to examination required by section one hundred and fifty-two. He shall pay to the committee at the time of his filing such application a fee which shall be determined annually by the commissioner of administration under the provisions of section three B of chapter seven.
