Annual reports shall be filed with the State Board of Health by the ambulance service permitted to conduct and implement a membership subscription program in the manner and form prescribed by the State Board of Health, which report shall contain the following:
The name and address of the ambulance service conducting the program;
The number of members subscribing to the subscription program;
The revenues generated by subscriptions to the program; and
The name and address of the depository bank in which the reserve fund is deposited and the amount of deposit in said reserve fund.
