An application for renewal of registration of a limited liability partnership must be executed and delivered to the secretary of state. The application must include but is not limited to the following information:
(1) the complete name and business mailing address of the limited liability partnership;
(2) the name and business mailing address of each partner; and
(3) a description of business being transacted.
