N.H. Rev. Stat. Ann. § 313-A:16

Applicants shall make written application to the office of professional licensure and certification on a form prescribed and supplied by the office which shall contain satisfactory evidence of the qualifications required of the applicant; and the applicant shall also pay the examination fee established by the office.
