N.H. Rev. Stat. Ann. § 436:11

Such permission shall be in the form of a written or printed permit and shall state the name and address of the importer, the number of such animals to be imported, the name of the place where such animals are to be held in quarantine for examination, and such other particulars as shall be deemed necessary.
