“Premium” means the consideration for insurance, by whatever name called. The term includes any “assessment,” or any “membership,” “policy,” “survey,” “inspection,” “service” or similar fee or other charge assessed or collected by the insurer or an agent of the insurer in consideration for an insurance contract or its procurement.
