Form of acknowledgment

R.L.1910, § 1179

An acknowledgment by individuals of any instrument affecting real estate shall be in substantially a form as provided for in the Uniform Law on Notarial Acts or in substantially the following form: State of Oklahoma,)) ss. __________ County.) Before me, ____ in and for this state, on this ____ day of ____, ________ personally appeared ________ to me known to be the identical person(s) who executed the within and foregoing instrument, and acknowledged to me that ____ executed the same as ____ free and voluntary act and deed for the uses and purposes therein set forth.
