Okla. Stat. tit. 74, § 3105

Information to be included in certain reports

A. Unless otherwise provided by law, every agency, department, board, commission or institution of the State of Oklahoma shall list the following information at a prominent place near the beginning of each publication issued by them:

1. Name of the issuing agency, department, board, commission or institution;

2. Authorization for publication. If such publication is not specifically authorized by statute the name of the person or persons so authorizing shall be stated;

3. The number of copies printed or identification of the website or websites at which the publication is located;

4. If applicable, the name of the printing firm doing the printing; and 5. Assurance of compliance with Section 3-114 of Title 65 of the Oklahoma Statutes. B. The information shall be set forth in a separate paragraph and shall conform as nearly as practical to the following applicable format:

1. If the publication is printed: "This publication, printed by (name of printing firm) is issued by (here list the agency, department, board, commission or institution) as authorized by ______________. ______ copies have been prepared and distributed at a cost of $_______. Copies have been deposited with the Publications Clearinghouse of the Oklahoma Department of Libraries."; or 2. If the publication is located online: "This publication is issued by (list the agency, department, board, commission or institution) as authorized by _______ and is located at the following website(s): _________. This publication has been submitted in compliance with Section 3-114 of Title 65 of the Oklahoma Statutes." C. State promotion and informational publications produced by the Oklahoma Tourism and Recreation Department, Travel Promotion Division, the Commissioners of the Land Office and the Oklahoma Department of Commerce shall be exempt from the provisions of this section.
