The Oregon Health Authority may condition, suspend, revoke or refuse to renew accreditation of a radiology inspector for the following reasons:

(1) Knowingly falsifying information included on the inspection report form supplied by the authority.

(2) Substantially failing to comply with authority procedures.

(3) Failing to meet authority accuracy requirements.

(4) Such other grounds as the authority may establish by rule.
