Tenn. Code Ann. § 55-50-406

Application for commercial driver license

Known as the Uniform Classified and Commercial Driver License Act

The act spans §§ 55-50-101 to 55-50-902 (79 sections).

(1) The application for a commercial driver license shall include the following: The full legal name and current legal residential address of the person, including the street address and number or route and box number (or post office box number if the person has no bona fide residential street address);

(2) A physical description of the person including sex, height, weight, hair and eye colors;

(3) The applicant's social security number;

(4) Date of birth;

(5) Signature;

(6) Photograph;

(7) Certification that all information is correct;

(8) Statement that the applicant has read and understands the Tennessee Financial Responsibility Law of 1977, compiled in chapter 12 of this title; and

(9) Any other information deemed necessary or required by the department or the secretary of transportation.

(10) The application shall be accompanied by a nonrefundable application fee of six dollars ($6.00) that shall constitute expendable receipts of the department.
