Notice of expiration — Renewal

(1) The board shall notify each person registered under this part of the date of expiration of the person's certificate and the amount of the fee required for its renewal. The notice shall be sent at least sixty (60) days in advance of the expiration of the certificate.

(2) It is the responsibility of any person registered under this part to notify the board, in writing, of any change of address. Persons registered under this part may notify the board by e-mail of a change in their address.

(3) Certificates of registration shall be subject to late renewal for a period of one (1) year following their expiration date by payment of the renewal fee, plus a penalty in an amount set by the board, for each month or fraction of a month that elapses before payment is received. Any person whose registration has expired for more than one (1) year shall make application for registration; provided, that the board may, in its discretion, waive any further examination of such an applicant.
