INDIVIDUAL APPLICATION AND CERTIFICATE NOT REQUIRED

(a) An individual application from an insured under a blanket accident and health insurance policy is not required.

(b) An insurer is not required to provide a certificate to each insured under a blanket accident and health insurance policy.
