Wis. Stat. § 221.0201

Any number of adult residents of this state, not less than 3 nor more than 20, who desire to associate for the purpose of organizing a banking corporation under this chapSTATE BANKS 221.02053 ter, may apply to the division to organize a bank. The application shall be made on a form prescribed by the division.
