Issuance of Licenses

To obtain an original applicator’s license, an applicant shall submit to the registrar an application in writing containing a statement that the applicant requests the issuance of an applicator’s license under the terms of this chapter.

The application shall be made on a form prescribed by the board and issued by the registrar in accordance with rules and regulations adopted by the board and shall contain the following:
