The ballot shall contain the words “Boulevard District—Yes,” and “Boulevard District—No,” and shall also make provision for voting for one member of the commission.
The ballot shall contain the words “Shall the boulevard district be formed?” To the right of or below these words, the words “Yes” and “No” shall be printed on separate lines, with voting targets. The ballot shall also make provision for voting for one member of the commission.
