Licensure by endorsement; requirements; fees

The department shall issue a license by endorsement to any applicant who, upon applying to the department on forms furnished by the department and remitting a fee set by the board in an amount not to exceed $500, the board certifies has met the requirements for licensure by endorsement under s. 456.0145.
The department shall issue a license by endorsement to any applicant who, upon applying to the department on forms furnished by the department and remitting a fee set by the board in an amount not to exceed $500, the board certifies has:
(1) Met the requirements for licensure by endorsement under s. 456.0145; or
(2) Met the requirements for licensure by endorsement under s. 456.0145 except for s. 456.0145(2)(a)4. but has submitted evidence to the board’s satisfaction of the successful completion of either a board-approved postgraduate training program within 2 years preceding the filing of an application or a board-approved clinical competency examination within the year preceding the filing of an application.
