(1) Any person who desires to be a certified public weigher in this state shall apply for and obtain a license permit from the Commissioner by filing a formal application as follows: I, ______________, a citizen of the United States, residing at ______________, County of ______________, having familiarized myself with the law relative to licensing of certified public weighers, do hereby make application for license permit as a certified public weigher.
