Iowa Code § 490.121

As of January 1, 2012
1. The secretary of state may prescribe and furnish on request forms including but not limited to the following:
1. a. The secretary of state may prescribe and furnish on request any of the following forms:
a. A foreign corporation’s application for a certificate of authority to transact business in this state.
b. A foreign corporation’s application for a certificate of withdrawal.
(1) An application for a certificate of existence or certificate of registration.
c. The biennial report.
(2) A foreign corporation’s registration statement.
(3) A foreign corporation’s statement of withdrawal.
(4) A foreign corporation’s transfer of registration statement.
(5) The biennial report required by section 490.1621.
If the secretary of state so requires, use of these listed forms prescribed by the secretary of state is mandatory.
b. If the secretary of state so requires, use of the forms provided in paragraph “a” is mandatory.
2. The secretary of state may prescribe and furnish on request forms for other documents required or permitted to be filed by this chapter but their use is not mandatory.
