Protocol for pharmacists

(1) must be agreed upon by: (A) the physician or the physician administrator described in section 3.5(d) of this chapter; and (B) the pharmacist; (2) must, at a minimum, require that: (A) the medical records of the patient are available to both the patient's physician and the pharmacist; and (B) the procedures performed by the pharmacist relate to a condition for which the patient has first seen the physician or another licensed practitioner; and (3) may apply to a single patient or group of patients, as specified by the physician.
