Declination to register

Sec. 6. An applicant who fails to check either box described in section 4(3) of this chapter or to sign a separate declination form under section 5 of this chapter shall be considered to have declined to register.
Sec. 6. An applicant who fails to check either box described in section 4(a)(3) of this chapter or to sign a separate declination form under section 5 of this chapter shall be considered to have declined to register.
