In addition to any other information that the Commission requires, the annual report of a public service company shall state:

(iv) bonds and other evidence of indebtedness issued and outstanding;

(6) when applicable: (i) a monthly compilation of expenditures that were the basis for any fuel rate adjustment; and (ii) an annual recapitulation of the information required under subitem (i) of this item.
