Notice of transitional services for incarcerated individuals released from correctional facilities

§ 76. Notice of transitional services for inmates released from\ncorrectional facilities. 1. Prior to the release of an inmate from a\ncorrectional facility, the department shall provide such inmate with\ninformation on transitional services available in the county or city\nwhere such inmate is scheduled to be released. Such information shall\ninclude programs designed to promote the successful and productive\nreentry and reintegration of an inmate into society including medical\nand mental health services, HIV/AIDS services, educational, vocational\nand employment services, alcohol or substance abuse treatment and\nhousing services. The department shall maintain a current list of\ntransitional services which shall be updated regularly in order to\neffectuate the purposes of this section. Where appropriate, the\ndepartment shall provide assistance to an inmate in contacting a program\nor service provider prior to such inmate's release to the community.\n 2. The commissioner shall consult with, and be entitled to receive the\nassistance of, the commissioners of education, labor, health, mental\nhealth and the office of temporary and disability assistance in the\nimplementation of this section.\n
§ 76. Notice of transitional services for incarcerated individuals\nreleased from correctional facilities. 1. (a) Prior to the release of an\nincarcerated individual from a correctional facility, the department\nshall provide such incarcerated individual with information on\ntransitional services available in the county or city where such\nincarcerated individual is scheduled to be released. Such information\nshall include programs designed to promote the successful and productive\nreentry and reintegration of an incarcerated individual into society\nincluding medical and mental health services, HIV/AIDS services,\neducational, vocational and employment services, alcohol or substance\nabuse treatment and housing services. The department shall maintain a\ncurrent list of transitional services which shall be updated regularly\nin order to effectuate the purposes of this section. Where appropriate,\nthe department shall provide assistance to an incarcerated individual in\ncontacting a program or service provider prior to such incarcerated\nindividual's release to the community.\n (b) Upon discharge of an incarcerated individual from a correctional\nfacility, the department shall provide such incarcerated individual with\neducational information about the prevention of human immunodeficiency\nvirus (HIV) infection, instructions about how to obtain free HIV testing\nupon release, including contact information for HIV counseling and\ntesting service providers located in the county or city in which such\nincarcerated individual intends to reside upon release, and referrals to\ncommunity-based HIV prevention, education and counseling resources\nlocated in the county or city in which such incarcerated individual\nintends to reside upon release.\n 2.
