* § 29. Intake forms for admission and residency. 1. The department,\nin cooperation with the office of temporary and disability assistance\nand any other state department, office, division or agency the\ndepartment deems necessary, shall require that all intake forms for\nadmission or residency to any temporary shelter that is reimbursed from\nstate or state-administered grants or funds shall ask an applicant:\n"Have you or anyone in your household ever been in the United States\nmilitary?". Each social services district or social services district's\ndesignee shall in writing advise all individuals applying for temporary\nhousing assistance and identifying themselves as having been in the\nUnited States military that the department of veterans' services and\nlocal veterans' service agencies established pursuant to section\nfourteen of this article provide assistance to veterans regarding\nbenefits available under federal and state law. Such written information\nshall include the name, address and telephone number of the New York\nstate department of veterans' services, the nearest department of\nveterans' services office, the nearest county or city veterans' service\nagency and the nearest accredited veterans' service officer. Each social\nservices district or social services district's designee, with the\npermission of such individual's identifying themself as a veteran, shall\ntransmit such veteran's status information to the department of\nveterans' services.\n 2. The department, in cooperation with the office of temporary and\ndisability assistance and any other state department, office, division\nor agency the department deems necessary, shall encourage all other\ntemporary shelter providers to share information to increase veteran\naccess to benefits by:\n (a) providing information on the department website including:\n (i) potential questions for inclusion on intake forms including, but\nnot limited to: "Have you or anyone in your household ever been in the\nUnited States military?";\n (ii) advising such providers that all individuals identifying\nthemselves as having been in the United States military that the\ndivision and local veterans' service agencies provide assistance to\nveterans regarding benefits available under federal and state law; and\n (iii) the address and telephone number of the department, county and\ncity veterans' service agencies and accredited veterans' service\nofficers; and\n (b) facilitating the transmission of such veteran's status\ninformation, with the permission of individuals identifying themselves\nas a veteran, to the department.\n * NB Effective April 1, 2023\n
