Mandatory forms and filing methods

(1) Mandatory Forms. The secretary of state may prescribe and furnish on request forms for: Articles of organization;
(1) Mandatory Forms. The secretary of state may prescribe and furnish forms and filing methods for all filings required by this title.
(2) Certificate of conversion;
(3) A foreign LLC's application for a certificate of authority to transact business in this state;
(4) A foreign LLC's application for a cancellation of a certificate of authority; and
(5) The annual report.
(6) If the secretary of state so requires, use of these forms shall be mandatory.
(2) If the secretary of state so requires, then use of these forms and filing methods is mandatory.
(7) Optional Forms. The secretary of state may prescribe and, if prescribed by the secretary of state, shall furnish on request forms for other documents required or permitted to be filed by chapters 201-248 of this title but their use is not mandatory.
