(1) A petition under this part may be filed at any time before entry of a decree of adoption.

(2) The petition shall be signed and verified by the petitioner, filed with the court, and state: the name and address of the petitioner;

(4) the name and last known address of the parent or alleged parent whose parental relationship the petitioner seeks to terminate;

(5) the facts and circumstances forming the basis for the petition and the grounds on which termination of a parental relationship is sought;

(6) if the petitioner is a prospective adoptive parent, that the petitioner intends to proceed with the petition to adopt the minor if the petition to terminate is granted; and
